NOTICE OF FORM 1095-C AVAILABILITY

Pursuant to the Paperwork Burden Reduction Act (H.R. 3797), this notice is provided to inform you of the following:

The Form 1095-C will only be made available to employees through request.

Beginning with tax year 2024, employers are no longer required to automatically provide employees with Form 1095-C (proof of employer offered health insurance coverage) unless specifically requested by the employee. The Form 1095-C will still be processed and reported by the company to the IRS.

Timeline for receiving your form

Once you have completed the request your Form 1095-C will be mailed within 30 days.
